Mary Jo Fuentes-Leste

As owner of Access TLC Hospice Foundation, Mary Jo Leste and her business partner, Shelley Chilton, saw the lack of continuity of care for patients at the end of life.  Twenty-two years ago, they began Access TLC Health Care which offers Hospice, Home Health, Caregiving,  Skilled Nursing Facility, Transportation, and the Foundation.

Mary Jo is deeply committed to her community and demonstrates an amazing care, passion, and service to others.  She grew-up in Mexico City, Mexico, is fluent in Spanish, and deeply loves working with and assisting the Hispanic community.

Mary Jo is a former Board member for Los Robles Regional Medical Center.  She is past President and Vice President for the Ventura County Home Care Association, serving in those capacities for 6 years.  Currently, Mary Jo is active on the La Reina High School Parent Guild, Crespi Carmelite Fundraising Committee, the Education Committee at St. Paschal Baylon Church, and an active Board Member for the Loving Heart Hospice Foundation.

In her “spare” time, Mary Jo loves to dance, swim, and travel with her husband Michael Leste and two children, Connor and Jade.

Charity: Access TLC Hospice Foundation

Access TLC Hospice Foundation provides financial support for medical needs, home care, hospice care or burial assistance to those with low income or financial need so that they have ‘quality of life at the end of life.’

Created in 1994, Access TLC Hospice Foundation discovered that many terminally ill patients in our community needed support, so the foundation was formed.

Support from the Foundation may include:

  • Caregiver Relief
  • Respite care in a skilled nursing facility
  •  Grief counseling
  •  Medications not covered by insurance
  • Burial assistance
  • General hospice education and continuing education for medical staff

The Foundation’s primary funding comes from individual donations, grants from public and private foundations, and our annual signature fundraising events; Kentucky Derby Gala, Golf Tournament, and many others.

Support local community based nonprofits & those in your community today—by giving—so that we may provide ‘Quality of Life…at the End of Life!’ Thank you for making a difference in the lives of others!
